2025-2026 AIA Physical Paperwork (For Doctor)

All physical packets will be done online at Register My Athlete.  This is the same site that the high schools use.  Parents will need to have their student athletes registered on this site before they can try out.  The four AIA physical pages for the doctor are below and will be available next school year in the front office.  These forms can be uploaded to Register My Athlete.

Family Pass:  $30

Family passes are used for cross country, baseball and softball.  This allows you and your family and friends to enter baseball and Softball games and cross country meet.  You purchase one card for the season.  

Punch Card: $30

Punch cards are used for volleyball, cheer and basketball.  The punch card gives you 10 games for $3.00 a game.  You can use punches for multiple people to get into a game.  One punch for each person.
